Diablo Immortal Flesh Harvest: New Quest, Gems & Strife Rework

Diablo Immortal's next major update, Flesh Harvest, expands the Sharval Wilds, adds three Legendary Gems, rebuilds major parts of the Cycle of Strife, improves Familiars and Warbands, and introduces custom PvP through War Games.
The new hunt for Albrecht leads the patch, but long-term players may feel the Shadow War and Clan progression changes more often than the quest itself.
Blizzard lists maintenance for September 24, 5–7 p.m. PDT for Oceania, East Asia, Southeast Asia, and Europe, followed by September 25, 1–3 a.m. PDT for the Americas. Features without a separate date are scheduled to become available after each region's maintenance finishes.
Flesh Harvest pushes deeper into the Sharval Wilds
The new main quest sends players into the Wicker Fields, the spiritual center and agricultural heart of Sharval, while the search for Prince Albrecht and his followers continues.
The journey crosses corrupted farmland filled with undead, frenzied fey, and Shardborne enemies before reaching the Boil of Creation, a huge hive tied to Albrecht's plans.
Several Sharval sub-zones also open for exploration, bounties, and wanted monsters, including Highfork, Orthrup, Moonlit Highlands, Darkened Glade, and Wicker Fields. Previous quests can be replayed through the Adventurer's Chronicle in Cain's Workshop.
This follows September's Made to Suffer update, which expanded the Lut Gholein storyline and opened the High Ward. The code can be claimed once per account for 1 Legendary Crest, 2 Legendary Items, 3 Aquamarines, and 3 Sapphires.
Three new Legendary Gems enter the pool
Flesh Harvest adds one new gem at each rarity tier.
Haunt Coil — Five-Star
Haunt Coil has a 10% chance to haunt an enemy for six seconds when you deal damage.
Its effect changes depending on stance. In Offensive Stance, haunted targets take increased damage from you. In Defensive Stance, they deal reduced damage to you. Haunted enemies also release periodic blasts that damage nearby enemies.
The same target cannot be haunted again more often than once every 20 seconds. Its value will depend heavily on build and content; the published values are Rank 10 values.
Fading Nostrum — Two-Star
Fading Nostrum activates after taking damage and grants a six-second ward. The ward reduces damage from harmful continual-damage effects and increases the chance to Block projectiles.
It has a 20-second internal cooldown.
It is a defensive gem rather than a universal damage upgrade.
Mountain Toe — One-Star
Mountain Toe increases Critical Resistance while reducing Critical Hit Damage taken.
It is the simplest of the three new gems and is clearly aimed at defensive consistency rather than adding another active proc to track.
Shadow War is moving away from elimination brackets
The largest structural change in Flesh Harvest is the Cycle of Strife rework.
Shadow War is switching to a points-based format instead of a traditional elimination bracket. Clans now play two matches per week and earn:

1 point for a loss
Clans are matched against opponents with similar point totals as the tournament develops. A previous matchup is also less likely to repeat immediately.
The top 10 Clans receive stronger rewards, while Clans finishing 11–20 still receive rewards. The top 10 earn Legendary Crests for members, while ranks 11–20 receive a Legendary Crest Voucher that can be exchanged through the Hilts Trader within the stated deadline.
Registration runs from Monday at 3 a.m. to Tuesday at 9 p.m. local server time, with 30 players required. One bad matchup no longer instantly ends the run.
Objectives should take longer to melt
Shadow War's battlefield is also being adjusted.
Building and minion health is increased, with health scaling based on the number of players in the match. Blizzard says this is intended to roughly double the time needed to destroy them.
Buildings also deal much more damage:

Outer Towers: 20% of player Health per attack

Inner Towers: 30%

Core/Crystal: 50%
Attacking buildings no longer grants Lifesteal, and Lifesteal against Sacred Guardians is reduced to 50% effectiveness.
That should make objective rushing more dangerous and reward better coordination.
Shadow Contracts are shorter but pay more
The rework also cuts some of the routine grind.
Weekly Shadow Contract attempts drop from nine to six, Familiar AFK time is reduced from 162 hours to 108 hours, and the cumulative attempt cap is removed. Contracts become available twice each week.
At the same time, activity points per completed activity rise from 20 to 30, while Gold, XP, and equipment from Contract rewards are increased by 50%.
The goal is less repetition without reducing the reason to participate.
Clan progression gets new rewards and a new fight
Clan levels can now climb higher through activities such as Vault Chests, Contracts, Shadow War, and Invasion of the Immortal Realm.
At specific milestones, Shadow Clans can unlock Legendary Shadow Cloaks. These must be re-earned each season, so they are seasonal rather than permanent unlocks.
Flesh Harvest also introduces Akeba's Ordeal, a four-player Shadow Clan fight against two bosses. Higher difficulties can add Challenge Dungeon Affixes, and the first completion each week provides increased rewards and Marks.
Shadow Assembly also speeds up, with blessings reduced from two-minute intervals to one.
Warbands and Vault runs get less friction
Warbands now need only four members for the highest-quality Remnant drops in Purge the Depths, while Remnants can also drop from Warband Helliquary Raids. Warband Rooms are removed so players can select their preferred role and active bonus stats more independently.
The Vault can also spawn a Hidden Passage that skips a party to the fourth floor while granting earlier chest rewards, at the cost of alerting Immortals and strengthening the Vault Guardian.
Familiar conversion is becoming less punishing
Familiar Conversion gets an important safety improvement.
After converting, players can choose between keeping one of the material Familiars or taking the new conversion result. That reduces the risk of sacrificing a valuable Familiar and ending up with something worse.
The Familiar Shop will show its next refresh time, and the first 10 seconds after a refresh use a bidding process. The post-trade freeze period is extended to seven days.
War Games adds custom PvP lobbies
Flesh Harvest also introduces War Games, a cross-server custom PvP system.
At launch, players can create lobbies for Destruction's Call — Capture the Flag, Destruction's Call — Slayer, and Tower War. Destruction's Call supports up to 12v12, while Tower War supports up to 8v8, with as many as 40 players in a lobby including spectators.
Hosts can adjust settings such as score requirements, kill targets, time limits, flags, and enemy spawns.
War Games uses the Strife debuff, which reduces advantages from Legendary Gems and other power sources, but Blizzard specifically notes that it is not a fully normalized fair-PvP environment.
What should you prioritize when Flesh Harvest arrives?
Start with the new quest if you follow Diablo Immortal's story, but Clan players should read the new Shadow War structure before the first registration window. The points system, objective durability changes, and revised rewards directly affect weekly planning.
Then redeem FLESHHARVEST once the code window opens, review the new Gems before spending resources, check Familiar conversion rules, coordinate Clan participation, and test War Games before treating it as serious competitive practice.
Battle Pass 44, Eyeless Omens, begins September 25 at 3 a.m. local server time and runs through October 23. The Festival of the Ancient Moon follows from October 1–15.
