Diablo Immortal Made to Suffer: Full Patch Notes & New Gems

Diablo Immortal's "Made to Suffer" update pushes deeper into Lut Gholein's ongoing Nation of Agony storyline, adding a new main quest, an entire new district, two new Legendary Gems, and a full reversal of last patch's Battlegrounds redesign. Maintenance runs September 16, 5–7 p.m. PDT for Oceania, East Asia, Southeast Asia, and Europe, and September 17, 1–3 a.m. PDT for the Americas — everything below goes live once that maintenance clears, with a few features on their own separate dates noted below.
Main Quest: Made to Suffer and the High Ward
The "Made to Suffer" quest continues the Nation of Agony storyline, pulling in allies from Diablo II's Act II as players push toward the source of Lut Gholein's twin threats. It's available to any player Level 60 or higher, though Blizzard recommends finishing "The Taking" and "Bloodied Jewel" first to avoid spoiling earlier story beats. Alongside the quest, the High Ward opens as a new district surrounding Sultan Jerhyn's palace — a wealthier commercial area still under partial demonic occupation, with its own bounties, objectives, and named wanted monsters.
City of Pain Event (September 17–30)
Running for two weeks after the quest drops, City of Pain has players complete activities across the new main story, Lut Gholein, Battlegrounds, and dungeons to earn City of Pain Tokens — up to 1,300 per week — which exchange for rewards in the Treasure of Hope, including Sacred Chests and Legendary Equipment. Blizzard designed the event's objectives to overlap with normal expansion progress, so most of the Token total should come from playing the new content naturally rather than requiring a separate grind.
Bloodied Jewel Replay and Hero's Journey Chapters 5–6
The "Bloodied Jewel" questline is now replayable through the Adventurer's Book at Cain's Workshop. Hero's Journey also unlocks two new chapters, gated sequentially — Chapter 4 must be finished before Chapter 5 progress counts, and Chapter 5 before Chapter 6:
- Chapter 5 rewards: 30 Aspirant's Keys, 900 Hilts, 1 bound Legendary Crest, 50,000 Gold
- Chapter 6 rewards: 36 Aspirant's Keys, 900 Hilts, 1 bound Legendary Crest, 1 Portal
Battlegrounds: Nephalem Theme Returns
Blizzard is walking back most — but not all — of the demonic invasion Battleground redesign. The visual theme reverts to the familiar Nephalem style, largely because the demonic environment's heavy red tone was making enemy indicators and skill effects harder to read mid-fight. Small Demons and their gates are removed entirely, since PvE enemies were unintentionally interacting with class mechanics inside what's meant to be a PvP-focused mode.
Two changes from the demonic invasion era are staying, though: the wider road layout is kept, since it improved positioning and combat flow independent of the visual theme, and the central map mechanic survives in adapted form — instead of defeating a Greater Demon to summon Nephalem Spirits, that same mechanic now ties into the Sacred Guardian instead.
Separately, Battleground season length is dropping from 10 weeks to 8 starting next season, on the logic that most players finish the bulk of their rank progression early anyway — shorter seasons mean more frequent rank resets and reward cycles across the year rather than a long tail where progression stalls out. Blizzard also confirmed Bout of Realms and Challenge of Equals dates are now available for the remainder of 2026, with the full 2027 tournament calendar promised early next year.
New Legendary Gems
Two new gems join the pool:
Enigma Latera (5-Star) — Periodically grants Enigma; your next Primary Attack then unleashes Cruel Enigma, chaining through up to 6 nearby enemies for 3 seconds. Cruel Enigma boosts damage dealt to afflicted enemies while reducing the damage they deal back, stacking up to 6 times per beneficial effect they carry. It also siphons a non-Skill beneficial effect from an afflicted player, granting you that same effect for its remaining duration. Enigma can't trigger more than once every 9 seconds. Compatible Resonance Gems: Tundra Blight, Fading Nostrum, Stubborn Oracle, Void Spark, Fated Trail.
Prong Crag (2-Star) — While in combat, taking a single hit exceeding 10% of your max Life grants Resolve for 6 seconds, increasing damage dealt and reducing damage taken from that source, stacking up to 3 times; it also raises Critical Resistance. Resolve can't trigger more than once every 20 seconds. Compatible Resonance Gems: Faltergrasp, Tundra Blight, Rampart Torch.
Players can trial a temporary Rank 10 version of Enigma Latera in dungeons, Elder Rifts, and zone activities for about an hour at a time, running September 17 through November 4. The Featured Gem Pool carries a 50% pull rate for Enigma Latera from September 17 through December 17, and both new gems join the Standard Pool as well. Separately, Normal Gems now get Progressive Upgrades starting at Rank 6, matching the incremental stat-gain system Legendary Gems already use, and the current Drop Pool window is extended from September 17 through December 17.
Free Skin: Ghost of Ashwold
As part of Diablo's 30th anniversary celebration, all players can claim the Ghost of Ashwold skin for free at the end of the event — Blizzard framed this directly as a response to player feedback that anniversary rewards should reach everyone, not just paying players.
Twisting Torments: New Weekly Zone Event
Twisting Torments is a new zone event in the Rocky Waste, just outside Lut Gholein, built around Andariel and Duriel each taking turns invading the city. Every Tuesday, one of the two bosses' influence takes over for the full day, spawning empowered minions with new affixes — Chains of Anguish under Andariel's influence, maggot-spawn from the Pain Burrows under Duriel's. Players destroy three altars to summon a Lieutenant; defeating it grants rewards, and defeating both Andariel's and Duriel's Lieutenants across their respective weeks unlocks increased rewards, including a weekly Legendary Equipment drop.
Quality-of-Life Improvements
A long list of smaller changes ships in the same patch:
- Iben Fahd's Sanctum now allows additional chest openings past the standard daily limit, scaled by server age — older servers get more, aimed at helping established players progress Legacy of the Horadrim faster.
- Eastgate Monastery gets new dialogue, with rogues suffering under Andariel's influence available to speak with.
- Phantom Market cosmetics move to a new standardized set of face templates as their base, meant to keep future cosmetic releases visually consistent.
- Player inspection now shows a profile's full cosmetics, portals, and special effects, not just gear.
- Main Quest Dungeons gain a difficulty-lowering option after repeated failures, letting players continue without restarting the instance from scratch.
- Event tab gets new categories and sub-tabs for easier navigation.
- Raid the Vault's Hidden Passage moves to a more visible mid-path location and no longer requires 4 players to activate — though full-party entry is still required.
- An engine update reworks roughly 100 affix effects, improves lighting and volumetric fog across more than 10 zones, reduces visual clutter, and enhances terrain and water textures (visual fidelity will vary by device).
Class Balance
Barbarian, Blood Knight, Crusader, and Druid are all getting balance adjustments with this patch; full specifics are posted on Blizzard's official forums rather than in the patch summary itself.
Bug Fixes
Several class-specific fixes round out the patch, including corrected missile tracking for the Demon Hunter's Covert Equalizer Essence during Strafe with Turmoil, a fix preventing Bloodscent's bonus damage from triggering without a Bleed-affected target, a corrected third-kick trigger for the Monk's Clashing Sky with Undue Censure, an updated (but functionally unchanged) description for the Druid's Eternal Cycle Essence, and a fix ensuring Tempest's Drenched effect properly applies to Bosses.
